간호학과 의학이 발전한 지난 수세기 동안 건강을 측정하는 기준으로 질병을 주로 사용하여 왔다. 하지만 건강을 이해하는데 질병에 초점을 맞추는 것은 문제가 있음이 여러 학자들에 의해 지적되고 있다. 질병중심적 시각의 이러한 문제점 때문에 간호학에서도 건강중심적인 논의가 확산되고 있다. 의료사회학자인 Antonovsky는 질병중심적인 시각을 대체할 수 있는 건강요인지향적 모델을 제시하고 있다. 그는 질병요인이 모든 사람에게 보편적으로 존재하는 것으로 보며 따라서 문제가 되는 것은 어떤 사람이 질병에 걸리는가가 아니라, 어떤 사람은 왜 보편적인 질병요인의 존재에도 불구하고 질병에 걸리지 않고 건강하게 지내는가이다. 그는 개인을 건강하게 하는 요인으로 전반적 내성자원(Generalized Resistance Resources)과 현실확신감(Sense of Coherence)을 들고 있으며, 또한 개인은 안락/불편(또는 건강/질병)의 연속선상에서 끊임없이 움직이고 있다고 생각하고 의료종사자가 할 일은 건강요인을 증진시켜 개인을 안락(또는 건강)의 방향으로 이동할 수 있도록 도와주는 것이라고 본다. 그의 이러한 이론은 질병중심적 모델을 보다 심각하게 반성해 볼 필요성을 제기한다. 또한 개인을 특정 질병에서 구제하는 이론들보다도 개인을 전반적인 건강상태에 있게 하는 이론과 방법들을 개발해야할 필요성을 제시하고 있다. 그의 이론은 간호학의 많은 내용을 혁신적으로 바꿀 수 있는 생각들을 포함하고 있다.
Disease has been used for criteria in understanding health in nursing and medi-
cine for several centuries. But some scholars point out that there is a problem to
understand health by disease. Nusring has also been based on pathogenic perspec-
tive in its knowledge development and practices.
Aaron Antonovsky, a medical sociologist in Israel, suggests the Salutogenic
Model to overcome the problems of pathogenic perspectives. According to him, an
appropriate question in health is not why someone gets sick but why someone
stays healthy while others do not. He thinks, since an individual is always moving
on the ease/disease continum, what health professionals have to do is to help
people move toward health(or ease). He emphasizes the importance of sense of co-
herence and generalized resistance resources of an individual in moving toward
health and suggests ideas for research.
His theory makes nursing science reflect on its traditional pathogenic orientation
more seriously. His ideas also stimulate nursing to develop theories and skills to
move people toward health. His ideas can dramatically change traditional nursing
education and practices.
